Your first appointment is an important step toward understanding and managing your condition. Here's exactly what happens and how to prepare.

A note from our staff

Dear New Patient,

Thank you for trusting the providers at Buckhead Rheumatology with your rheumatological care. At your initial visit, you will typically see both Rattandeep Singh, MD, and a physician assistant. For subsequent visits, we use a shared schedule, so you may be seen by any one of the three providers in our practice throughout your care.

We recommend completing your new patient paperwork before your appointment. If you are unable to do so, please arrive 15 minutes early to complete it in our office.

Complete your paperwork online

  1. 1Open Patient Forms and select the two-step online form.
  2. 2Complete Patient Registration and Medical / Social History as fully and accurately as possible.
  3. 3Print both completed forms and bring them to your appointment.

Additional forms requiring your signature will be available through MyChart when you prepare for your visit with Rattandeep Singh, MD.

Please bring to your appointment

  • A list of all medications, including dosing instructions and your pharmacy's name and phone number.
  • Any referral required by your insurance policy.
  • Your insurance card and photo ID, plus payment for any applicable deductible, coinsurance or copay.
  • Pertinent medical records, including lab tests and imaging reports such as X-rays, MRIs, CT scans and bone density scans. Whenever possible, ask your referring physician to send these before your visit.

Parking

The building's parking deck accepts credit cards only. Buckhead Rheumatology does not validate parking.

MyChart patient portal

We use Piedmont Hospital's secure MyChart portal. Once established, your account lets you manage appointments, request medication refills, view lab results and exchange secure messages with our staff.

Cancellations and missed appointments

Please notify us at least 24 hours in advance if you cannot keep your appointment. We reserve the right to charge $60 for a missed appointment. This fee is not covered by insurance and must be paid before your next appointment.

  2. 02Diagnostic testing, if neededBased on your initial evaluation, your rheumatologist may recommend blood work or imaging studies to gain more insight into your condition.
  3. 03A personalized treatment planWith all the information gathered, your rheumatologist works with you to build a plan that fits your specific needs and your life.
